Home Care Registered Nurse Part Time ($27.50-$33.00)
Our Community Support Services Team provides every level of services and care throughout all of our Communities in the Denver Metro Area, as well as in residents private homes.

Schedule: Monday - Friday 8:00am - 1:00pm.
Wage Range: $26.50 - $31.80 Based on Experience
Position Summary: This position is responsible for providing professional clinical home care services for clients. Responsibilities include performing direct skilled nursing care, admissions, comprehensive assessments, home visits related to care or supervision, medication management and care plan updates. Evaluates client needs for other services, making recommendations when appropriate. Coordinates and confirms scheduling of client services. Performs scheduled and unscheduled visits as necessary and communicates plan of care with other team members. Submits accurate and timely documentation. Assists with supervision and direction of care by C.N.A.’s and LPN’s. Communicates regularly with team members.
Basic Requirements & Experience Needed:
Colorado Registered Nursing License, in good standing.
Minimum of 2 years experience in a home health environment.
Must have a current Colorado driving record in good standing.
Current CPR certification
Strong knowledge of governmental home health regulations.
Excellent assessment and communication skills required.
Minimum 1 year experience supervising direct reports preferred.
Must be able to read, write and speak the English language.
Must possess excellent customer service skills.
Proficient in Microsoft Office applications including Excel, Word and Outlook
Essential Duties of the Home Care Services Registered Nurse:
Performs skilled home health care to clients, following physician orders and in coordination with other team members.
Coordinates client care with case managing team members and Clinical Director. Evaluates client nursing needs and develops a plan of care per physician orders.
Responsible for setting up medicinal reminders for clients.
Responsible for training and supervising home health C.N.A.’s and L.P.N.’s.
Continually monitors and assesses client’s status and reports changes to appropriate personnel.
Participates in in-services, meetings, and care conferences.
Instructs client and family in the plan of care, and refers to community resources as necessary.
Revises and updates nursing plan of care as appropriates, with physician approval.
Promotes customer satisfaction for all clients.
Completes 12 topics of in-service training per year.
Participates in on-call rotation schedule
Other duties as assigned.
Working Conditions and Physical Demands of the Job:
Moderate physical activity required by handling objects up to 50 pounds occasionally and/or up to 20 pounds frequently.
Exposure to blood borne pathogens, body excretions and communicable diseases possible
Sits, stands, bends and moves intermittently during working hours.
May be subject to frequent interruptions.
Interacts with clients, family members and visitors frequently.
