Social Services Director

Generally Monday - Friday 8:00 a.m. - 4:30 p.m., this full time position will qualify you for our full time benefit package, which includes a robust Paid Time Off plan, as well as having a majority of your Health and Dental Plan premiums paid.

Salary Range: $25.67 - $29.15 / Negotiable, based on years of experience and degree.

Position Summary

Provides comprehensive psychosocial support for residents and their families in our Health Suites (SNF). We prefer an experienced Social Worker in a skilled or hospital environment. Must have excellent communication skills, be highly organized, and relate to residents, families, and fellow associates. Must have full knowledge of the PASSR system and process, in addition to reimbursement, MDS and care planning. Must be compassionate about empowering the residents in our Eden environment.

Essential Duties:

  • Responsible for assessment, intervention and evaluation of resident’s psychosocial needs.
  • Provide counseling and emotional support to residents and family members.
  • Responsible for all resident documentation relating to Social Services including completion of the MDS, social histories, anecdotal charting, and other related information
  • Responsible for identification, intervention and documentation of resident with special needs
  • Discharge planning.
  • Disseminates information regarding available financial resources and helps maintain and insure continuation of any monetary benefits that residents are entitled to and/or eligible for.
  • Other duties as assigned.

Basic Qualifications & Experience

  • At minimum a Bachelor of Science in Social Work or equivalent as allowable by regulatory standard.
  • A minimum of 2 successful years’ experience as a Social Worker in a long-term care facility.
  • Must possess excellent communication skills
  • Must be able to successfully work independently and on a team
  • Must have functional knowledge and the skills to guide residents and families through the Medicaid, Medicare, and other insurance processes.  Must have established skills with the PASRR and PETI programs, as well as the Medicaid application process.
  • Computer competency with ability to use basic Window products and electronic health record.  MDS a plus.
  • Must be able to read, write and speak the English language.
  • Must be able to effectively gather a network of any outside agencies or services to assist with meeting the needs of our residents and families
  • Responsible for coordinating ancillary services with internal and outside providers (dental, optometry, referrals, etc.).

Working Conditions/Physical Requirements

  • Light physical effort required by handling objects up to 20 pounds occasionally and/or up to 10 pounds frequently.
  • Sits, stands, bends and moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Interacts with residents, family members, visitors frequently.
